Health Statement

So, my recruiter got all of my documents, but apparently I didn't send the correct health statement form. I have never seen the one she just sent me. Grr... Oh well, easy fix. I'll fill one out, fax to e-mail it to myself and then send it to her. Then, maybe soon we can have everything completed.. and be on track to get our visa's!
